Some of these are specifically called out elsewhere in the rules; I didn't go look to get the rule number, but one example is repeated requests for time-out at the wrong time, or after a team has already called time-out.

Correct me if I'm wrong, Susan, but I think that's the wrong thing - I think we're talking about the five (ACF) / two (NAQT) seconds permitted to begin an answer after signalling. That rules citation is about a tie between the buzzer and moderator, not between the beginning of an answer and the mode...
